Here at Community Access Network, we offer primary medical care, regardless of age, income, or insurance status. Our team of skilled, compassionate, and knowledgeable providers, nurses, and staff help patients get the best, most appropriate health care services available.
If a patient requires additional specialized care, we maintain a network of trusted specialists and can assist in coordinating your referrals. Our primary care physicians strive to provide services and connect patients with whichever specialists they need to support their physical, mental, and emotional well-being.
Our primary medical care services include:

- Preventative care, such as well-child visits
- Routine check-ups and screenings
- Chronic disease management
- COVID-19 testing and vaccines offered as part of any visit
Preventive care helps our patients achieve and maintain better health over their lifetime. This includes vaccinations and immunizations for both adults and children, such as those that protect them from the flu. It also includes input and consultations from our health care team on how to develop habits and routines to achieve a more healthy lifestyle, how to quit smoking, and weight management assistance.
